Looking for a book title

I am looking for the title to a book I read in junior High. I remember one of the main characters was a young boy trapped in a military instalation during a nuke attack. he became a general named Roland ( I think ). Most of the people in the story ended up getting these cacoons on their heads and when the cacoons came off their true self was unveiled. I know this is vague but any help would be appreciated.
