Need help with listing the typos / grammar errors in the review

Hi, can someone please help - what is the exact format for writing the errors found in the book? My reviews are getting rejected because of this.
Is there any OBC documentation regarding this which specifically mention the format? Can you please explain along with an example?

Hi. I think there's no exact format. The most important thing is the page number being included in reporting errors, and it is being quoted.
When I report my errors. I wrote like this.
Error 1: grammar
Page 75- "We've have been working on this problem for a long time."
Proposed correction:
Page 75- "We have been working on this problem for a long time."
And in my previous reviews, it was okay, and having no issues with it.
