Help me find the name of a book

The book is about a person's journal about being in a psych ward. The pages have "hand written" pages in it as if the person was really keeping a scrap book. There are mock pictures in it too. The book is designed to look old and tattered.

I'm not sure if this is the book you're thinking of, but it could be "One Flew Over the Cuckoo's Nest" by Ken Kesey. The story is set in a psychiatric ward in Oregon State.
